Essentials Care Navigation Training – Delivered by the Primary Care Development Team

This training aims to provide a solid base knowledge of Care Navigation, what it is and how it benefits our patients and practice.  It also includes an overview of the ARRs staff in PCNs and details around population health needs.  This session is held monthly and is aimed at new starters or colleagues who would benefit from a refresher training session.  It will be hosted by the Primary Care Development team and will be an informal friendly space to ask questions and gain confidence when Care Navigating your patients.
